1. What documents should I submit if I wish to renew my passport?

The regular requirements for the renewal of your passport are:

A. Your original old or expired passport;

B. Accomplished passport application form;

C. Three (3) recent photos (45mm x 35mm) with royal blue background taken in a photo studio or professionally taken within the last six months before application; for the photo guidelines, click this link to the Philippine Department of Foreign Affairs' website, http://www.dfa.gov.ph you will find the appropriate links there;

D. Photocopy of passport data page;

E. Photocopy of permanent resident card, work permit or record of landing; and

F. The passport processing fee of US$ 50.00 (Tk 3,500.00).

If you’re holding a brown passport, you should submit the documentary proof of your complete middle name (original and one photocopy) in addition to requirements A to F. You may present your birth certificate, baptismal certificate, landing papers or your driver’s license as the documentary proof of middle name.

 

2. What should I do if I wish to change from my married name to my maiden name in my passport?

Only female applicants who have been widowed or whose marriage has been dissolved through annulment or divorce (provided the conditions surrounding the divorce are recognized under Philippine law) can apply for a change from married name to maiden name.

If the change of name applied for is a result of the death of the husband, the duly authenticated death certificate or the Court Order in case of presumptive death should be submitted in addition to requirements A to F.

If the application for a change of name is due to the dissolution of the marriage, a duly authenticated copy of the annulment/divorce decree or order should be submitted in addition to requirements A to F and the documentary proof of your middle name.

 

3. Can I choose NOT to use my husband’s surname in my passport?

You may choose not to use your husband’s surname in your passport provided that you have not used your husband’s surname in your passport before.

In this case, you should submit requirements A to H (with the documentary proof of middle name if you’re holding a brown passport), along with a duly-accomplished affidavit that you have opted to use your maiden name.

 

4. What documents does a dual citizen submit in order to apply for a new passport?

Applicants who have recently become dual citizens (by virtue of Republic Act 9225) and are applying for a new Philippine passport should submit requirements A, B, C, D, a photocopy of their Bangladeshi/Sri Lankan/Maldivian passport and citizenship ID, birth certificate, and marriage certificate (if applicable).

 

5. What should I do if I want to amend the entries in my current passport?

Female applicants who wish to change from maiden name to married name in their current passports should submit a duly-accomplished Request Form for Passport Amendment, requirements G and H, I, or J and their valid passport.

Female applicants who wish to change from married name to maiden name due to the death of the spouse should submit a duly-accomplished Request Form for Passport Amendment, their duly-authenticated Report of Marriage Form, their current passport, the duly authenticated death certificate or the Court Order in case of presumptive death, documentary proof of middle name, and the processing fee.

Female applicants who wish to change from married name to maiden name due to annulment or divorce (provided the conditions surrounding the divorce are recognized under Philippine law) should submit a duly-accomplished Request Form for Passport Amendment, their duly-authenticated Report of Marriage Form, their current passport, documentary proof of middle name, duly authenticated copy of the annulment/divorce decree or order, and the processing fee

 

6. What documents should a minor applicant submit to obtain a passport?

Minors (under 18 years) applying for their first passport should submit requirements B to F in addition to the following documents:
O. Personal appearance of the applicant and both parents;
P. Birth certificate;
Q. Marriage certificate of parents;
R. Letter from physician who delivered the child attesting to the delivery or medical record of the child from the hospital where he/she was delivered (if physician can no longer be located);
S. Original and photocopies of passport data pages of both parents; and
T. Letter of consent signed by both parents (Letter of consent must be notarized by a notary public or barrister, if only one or neither parent can appear in person)
U.  Accomplished Report of Birth form in triplicate

If a minor wishes to apply for the renewal of his passport, he should submit requirements A to F, S and T

 

7. What documents should I submit if I wish to apply for a replacement to a lost passport?

Replacements for lost or destroyed passports take fifteen (15) to thirty (30) working days to process. When applying for a replacement passport, you should submit requirements B to E, in addition to the following documents:

K. A Police Report or Case Certificate;
L. Your duly-accomplished Affidavit of Loss of Passport (may be accomplished at Philippine Embassy for separate fee);
M. Your duly-authenticated Birth Certificate and Marriage Certificate (if applicable), in addition to another legal document showing your name and date and place of birth; and
N. a photocopy of your lost or destroyed passport, if available.

You should also submit requirements H, I, or J, and the duly-authenticated death certificate of spouse or annulment/divorce order if these are applicable in your case.

 

 

8. Can I apply for a passport by mail?

No, all applicants whether for the renewal of a passport or those applying for the first time MUST appear personally before a Consular Officer to affix his thumb mark and sign his application form. His signature and thumbprint is stored in the new passport database. 

 

 

9. How do I pay for my passport fees?

The Embassy accepts only payment in cash for applications submitted in person. 

 

10. When will I receive my new passport?

The Consular staff will advise you of the date of release of your new passport.

Normally, however, in comparison to the green passports, there is a longer turn-around time of 30 days from date of application to the date when the applicant can receive his/her Machine Readable Passport (MRP).

At present, the passports are being produced centrally in the Philippines, by the Department of Foreign Affairs, Manila, unlike before when the green passports were being manually scripted and laminated in the Embassy.

The benefits of MRP definitely far outweigh the currently longer turn-around time.

 

In this regard, the Embassy advises all applicants to apply well in advance of their intended dates of travel.
